Nicholas Smith


Dr. Nick Smith is a clinical physical therapist treating patients at the Williamsville location of SCORE Performance Medicine.  Nick is a graduate of D’Youville and Canisius Colleges receiving a Doctorate of Physical Therapy and a Bachelor’s of Science in Athletic Training respectively. Nick maintains status as a Certified Athletic Trainer and a Licensed Physical Therapist in the State of New York.  Nick was previously employed by Rose Physical Therapy.

Nick’s passion for physical therapy was discovered during the course of education in the sports medicine field as an athletic training student at Canisius College. Nick’s initial interest in sports medicine led to an excitement in rehabilitation and a preference for a clinical setting, prompting him to attend D’Youville College and transition his education toward physical therapy.

Nick’s education and clinical experience in physical therapy have led to an interest in pain science, functional exercise, manual therapy, and continued interest in sports medicine. 

Nick has attended post graduate education surrounding treatment of the shoulder, spine, manual therapy, functional movement, and application of kinesiology tape.

On a day to day basis Nick’s treatment goals surround return to function and increased quality of life for each patient on a person to person basis. Nick is proud to offer physical therapy sessions heavy on analysis that stray from run-of-the mill treatments that lack individualization. Nick lives in the Town of Tonawanda with his wife and daughter.


Carley Whitford DPT

Dr. Carley Whitford is a clinical physical therapist treating patients at both the Lockport and
Williamsville Score Performance Medicine offices. Carley is a 2019 graduate from The
University at Buffalo’s Doctor of Physical Therapy program. Prior to receiving her DPT degree,
she completed an undergraduate degree in Exercise Science from UB in 2016.
Carley first became interested in the field of physical therapy when she shadowed at the Center
for Disability Services near her hometown. This made her realize what an impact functional
movement can have on someone’s quality of life, and peaked her interest in applying to UB’s
DPT program.
Carley is currently interested in functional movement training and manual therapy for the
orthopedic setting, as well as neurological rehabilitation for both the outpatient and inpatient
Outside of the clinic, Carley enjoys running, biking, hiking in the Adirondacks, and spending time
with family and friends at her favorite lake in Vermont.

Laura Murray


Laura is excited to join the team at Score Performance Medicine adding 24 years experience as a PTA. She is a focused clinician that enjoys having a career where she can motivate and help patients during their healing process. She ensures that each patient understands the treatment that has been designed specifically for them while maintaining the line of communication open between patient and therapist. She stresses the importance of continued education and has mentored many PTA students over the years.

Outside of the clinic, Laura enjoys spending time with her family. You will most likely find her on the hiking or biking trails, attending a sporting event, or at the beach. She is a member of Friends of Unified Sports, a proud mother, and loyal friend.


Shuyu Chang


Shuyu received a B.S. in Athletic Training from Canisius College in 2004 and an in PTA from Villa Maria College in 2006.  She works closely with our PT's to help patients achieve their goals through manual therapy and functional training.  She lives in Amherst with her husband, Tom, and their two beautiful daughters.


Jessica Schrauth, DPT

Dr. Jessica Schrauth joined the clinical team in June of 2020 providing clinical care as well as front desk support.

Jess is a Buffalo native and graduate of Hamburg High School.  She graduated from UB in 2017 with a B.S. in Exercise Science and minor in Nutrition.  She recently finished the DPT program at UB, graduating in May of 2020.  


Ginny Colopy

Medical Receptionist

Ginny is a medical receptionist and part of our front office team, assisting with the scheduling and front office desk duties.  She ensures that our Lockport office runs smoothly.  In her spare time, Ginny enjoys traveling and loves to cuddle with her yellow lab, Lily.


Lynn Engelhart

Medical Receptionist

Lynn is a Medical Receptionist serving patients in the Lockport office.  Lynn and her husband, Dale, are lifelong Niagara County residents.  They enjoy traveling to visit their children and grandchildren.  Her interests include local theater, playing Euchre, and sitting around a campfire.  She can be found in the summer enjoying RV life which she plans on doing more of when she retires.  Happy trails!


Jake Yaple



Jake graduated with his Bachelor's degree in Exercise Science from the University of Buffalo in 2012 and then went on to earn his Doctorate of Physical Therapy in 2015. He started his early career doing travel therapy with his wife in a variety of locations and settings across the country. After a couple years, he returned home to Western New York to put down roots in Pendleton. Jake has been working as a practicing Director of Rehab for Eastern Niagara Hospital since 2017. Jake's treatment philosophy involves using a combination of MDT, manual therapy, and functional exercises to optimize patient outcomes.
