Score Performance Medicine wants you to feel comfortable when you come in for your physical therapy appointment whether you are a new or returning patient. We want you to understand how we operate and what to expect during your physical therapy appointment. By using the button below, you will find a series of frequently asked questions. SETTING UP AN APPOINTMENT
If you are in need of a physical therapist, whether referred by a physician or using direct access, please call our conveniently located offices to set up your first appointment, an initial evaluation.
Please have your insurance information, script (if not using direct access), and calendar nearby.
Your initial evaluation will last between 1 and 1 1/2 hours so please schedule accordingly.
If you are scheduling a FCE (Functional Capacity Exam) with us we require payment up front or confirmed authorization from your insurance company before we can schedule you. A FCE will range between 2 and 3 hours.
